For your convenience prior to visiting our office, please click the links below to electronically fill out the New Patient Health History Form and the HIPAA Form for each child. An Additional Adult Consent Form is also available to fill out if anyone other than a legal guardian will bring your child to any of his/her visits (grandparent, stepparent, babysitter, etc..).
